Kakushu-shomeisho-hakko
(The Issuance of Various Certificates)

Applications for the issuance of various certificates, such as graduation certificates should be sent by registered mail for cash (genkin kakitome) with a completed Request for Certificate Issuance Form (attached below) enclosed alongside the documents below.Additionally, the issuance of certificates takes approximately two weeks, so please apply with ample time.

Documents necessary for certificate issuance

  1. Request for Certificate Issuance Form

  2. ID Verification Document (Please enclose a copy of any of the forms of ID below.)
    • Driver’s license、Driving Record Certificate (Issued on or after April 1, 2012)
    • Health Insurance Car (Please mask your insurer number and insurance identification code / number)
    • Passport
    • Certificate of Alien Registration (Alien Registration Card) *Must be within the validity period to be considered a residence card
    The various certificates issued will be issued in the name of the student at the time of enrollment. After graduation or removal of one’s name from the family register, the submission of the official copy of one’s family register is required to verify one’s changed family name.
  3. Issuance Handling Fees
    Graduates of the institute and academy must pay a handling fee of 300 yen (in cash) per certificate issued.
  4. Return envelope and stamps for postage
    In all cases please clearly specify your address, and affix postage stamps appropriate for the postage fees. The certificates will be sent folded to fit the size of the envelope. Please prepare a return envelope using the below as a guide. Please also confirm the details below in the “About postage fees” section for postage stamp guidance.
    • Nagagata 3 Gou (120mm×235mm) standard-sized envelope … An A4 paper can be folded in thirds and enclosed.
    • Kakugata 2 Gou (240mm×332mm) non-standard sized envelope... An A4 paper can be inserted without being folded. In the event that the envelope must be officially sealed, please use this size.

About postage fees

Regarding postage fees, please consider the weight of the return envelope as the institute will enclose and issue the Number of certificates to be issued, and Each transcript in an envelope provided by the institute.
If the return postage is insufficient for the enclosed weight / standard of the officially sealed envelope, please calculate and send additional stamps to make up for the shortage. Please be aware that until the missing amount is received that the certificates cannot be sent.

As a return envelope, the use of a “letter pack light” is permitted.

For persons who would like the return postage via express mail or recorded delivery, in addition to the postage fees, please affix the postage stamp for the relevant amount and on the top side of the return envelope please write in red “express mail” (速達) or “recorded delivery”(簡易書留). Please confirm any additional details with the postage office or with the Japan post homepage.

Points of caution

  • For the envelope addressing the university, please address it for the Educational Affairs Department(教務課), and write clearly that it is the Application for Certificate Issuance(証明書発行申請).
  • The issuance of certificates takes approximately two weeks, so please apply with ample time.
  • In cases where requested certificates are in English, on the Request for Certificate Issuance Form please append your name as written in Romaji (Romanized) in the name column too.
